Personal Enablement Program (PEP)
Silver Chain's PEP assists you to regain and boost your well being after a period of time in hospital. Often after being in hospital, you find it difficult to get back to independent living and PEP offers you advice and support to regain the skills to manage your own activities without the need for assistance. PEP is a home based program which operates across the Perth metropolitan area and in selected country areas. During PEP, you will be supported by the allied health team, who will work with you to identify the tasks you are having difficulty with and provide strategies to help you get back on your feet. You may also need help with your daily activities and this will be provided by our team of therapy assistants, care aides and home helps. PEP is a short-term service however if you need ongoing assistance after completing the program, we will assist you to transition across to other ongoing service options.

Home Independence Program (HIP)
HIP is an innovative service that aims to improve your overall health and independence if you are over 65 years of age. HIP is best suited to people who are starting to have difficulty managing their own activities at home, who have never had services before, or who need assistance to be more independent at home.
Activities include:
- Learning new ways of approaching personal and daily activities that have become more difficult, so that you can manage your own activities without the need for assistance.
- Using equipment that can assist with personal care and day to day activities.
- Safely regaining confidence with managing day to day tasks, especially after a fall or an illness.
- Learning how to reduce the risk of falling, both at home and outside.
- Providing a home exercise program that helps to improve strength, balance and increases overall physical activity levels.
- Education about healthy and successful ageing.
- Looking into community based activities that suit your individual interests.

Home Independence Program Manual
The Home Independence User Manual describes the development of the Home Independence Program (HIP) and how it works.
The HIP manual is currently out of print however it is intended it will be updated and reprinted in the future. If you would like any information about the HIP Manual, please email dwoods@silverchain.org.au.
