Silver Chain is funded by the Department of Health to supply oxygen to you in your home if you live within the Perth metropolitan area.
To have oxygen provided in your home, a referral needs to made. Referrals can only be accepted from the following sources:

Respiratory physician
-
Physician
-
Oncologist
-
Cardiologist
-
Haematologist
-
Neurologist
-
Geriatrician
-
Silver Chain's Hospice Care Service doctors.
Referrals can be made by fax 24 hours a day, seven days a week on (08) 9444 7265.

When a referral is made to Silver Chain a trained customer centre representative will process the request.
If you are eligible to receive oxygen in your home, we will contact you to arrange and deliver the oxygen to your home the next day. The oxygen unit consists of a concentrator and backup cylinder. The backup cylinder is provided for use only in a power failure.
Silver Chain is responsible for monitoring and reviewing your ongoing oxygen requirements. After one month of receiving oxygen in your home, you are required to be reviewed by the original referral source to reassess your needs.
