Assessment and client care co-ordination

Assessment and client co-ordination is provided by the Regional Assessment Service within the metropolitan area.  Silver Chain have Regional Assessment Services in all metropolitan areas.  Click here for more information.

Silver Chain supports people in the community with tasks that they are having difficulties with and where possible assist to strengthen their own abilites, such as (but in no way limited) to domestic assistance, personal care and social support.  If you are not a current client contact the Commonwealth Respite and Carelink Centre on 1800 052 222. In country Western Australia, Silver Chain staff will complete a comprehensive holistic assessment based on clients strengths and abilities to remain independent within their own homes.  Clients are re-assessed at least annually.  To make a referral to Silver Chain Country Services or to find out more on how we can help, contact 1300 300 122 for country callers (cost of a local call).
