Assessment and care co-ordination

Silver Chain supports people in the community with tasks that they are having difficulties with, in order to prevent further decline in their abilities and where possible, strengthen their abilities.

Silver Chain staff will complete a comprehensive holistic and strengths based assessment to determine which services are suitable to assist our clients to remain independent. Clients are assessed at least once annually.
