Aged care packages

Silver Chain is contracted to provide Aged Care Packages: CACP, EACH, EACHD, TCP.

Community Aged Care Packages (CACP), Extended Aged Care at Home (EACH) and Extended Aged Care at Home Dementia (EACHD) are flexible health care and support packages which enable people to remain in their own home when they would otherwise require admission to a residential care facility. 

CACP, EACH and EACHD offer holistic care delivered in your home and tailored through negotiation to meet your needs.  Silver Chain can also link you to other appropriate services and activities in the community.

To be eligible for CACP, EACH, or EACHD you need to be eligible to enter an aged care facility.  Therefore, your needs must be assessed by a member of the Aged Care Assessment Team (ACAT).  ACAT can meet with you in your own home or in hospital to determine whether CACP, EACH or EACHD is suitable.

Your local ACAT might include a doctor, nurse, social worker, occupational therapist or physiotherapist.

Examples of services available through CACP, EACH and EACHD include but are not restricted to:

  • Personal care such as bathing or dressing
  • Meals, laundry and short term respite
  • Transport, shopping and social activities
  • Housework, gardening and home maintenance
  • 24 hour on-call emergency assistance (EACH and EACHD only).

The advantages of CACP, EACH and EACHD are the flexibility designed to meet your individual needs.  These packages can assist you to maintain your independence with the security and support of your family and friends. To meet your specific requirements, the services can change as your needs change.
