Dementia Services
Silver Chain supports people living with dementia through home support services, day centres, residential and palliative care services. Most people living with dementia and their carers are successfully supported through regular Silver Chain services. Many Silver Chain staff complete a course in working with people with dementia as part of their regular training. Families can add to their Silver Chain service by attending events offered by other organisations such as Alzheimers Association of WA.
Silver Chain also provides a number of services that specialise in working with people with dementia and their families. These include, but are not limited to:
- Community Aged Care Packages and Extended Aged Care Packages specifically funded for people with dementia. Silver Chain offers these packages in some locations only. Other providers may provide a similar service in other locations.
- Specific residential services for people living with dementia.
Dementia Assessment and Intervention Specialists Initiative (DAISI)
This Silver Chain initiative brings together a multidisciplinary team, with specialist experience in dementia care to further improve services for clients living with dementia and their families. The DAISI team support clients and staff across Silver Chain with projects and services such as:
- Developing networks and links with other organisations.
- Producing easy to read information about dementia readily available to staff and families.
- Working directly with people with dementia who show behaviours of concern and who are referred by staff.
- Providing more advanced training to staff and advising of regular training opportunities.
- Initiating projects to further improve Silver Chain services for people living with dementia and their families.
- Participating in research and evaluations in order to find better ways of working.
We Always Dress for Sunday Lunch
‘We Always Dress for Sunday Lunch’ is an exciting, unique venture by Silver Chain to instil learnings about caring for people with dementia using the art of storytelling.
