Post Acute Care
Post Acute Care refers to services in the immediate post discharge period from hospital for a surgical or medical intervention.
Post Acute Care is nurse managed and can provide you with acute home care for a range of conditions.
A medical point of reference must be available for complications and this may remain with your general practitioner or consultant/specialist.
Post Acute Care provides services with the average number of days per episode being 14 days and a maximum of 21 days following an acute admission.
A registered nurse will determine the eligibility of all new referrals by discussing your care directly with the referrer.
If you have been an inpatient at the following hospitals or are a resident in the catchment area for the following hospitals, you may be referred to our Post Acute Care service:

King Edward Memorial Hospital (excluding obstetric care)
South Metro Region
Royal Perth Hospital
Fremantle Hospital
Kalamunda District Community Hospital

Rockingham-Kwinana District Hospital
Murray Districts Hospital
Goldfields Region
Kalgoorlie Hospital
Post Acute Care provides support with no direct cost to you, however you must be Medicare eligible.
