hospital@home
hospital@home is a program that provides home based acute care (short term care) to you, if you would otherwise be in hospital. The level of care is equivalent to being in hospital however, it is provided in the comfort of your own home. The service is available 24 hours a day, seven days a week.
You may be referred to the service if you need care for the following:
- Cellulitis.
- Deep vein thrombosis.
- Urinary tract infection.
- Kidney infection.
- Pneumonia.
- Or a condition that can be resolved in seven days.
Medical governance is provided by a doctor from the hospital@home program or by your referring hospital specialist. hospital@home nurses will provide the nursing care prescribed. Additional care such as physiotherapy can be organised if requested by your doctor.
Any supplies or equipment that is needed while you are receiving services through the hospital@home program will be supplied, as well as any medications or therapies administered by injection.
This service provides support with no direct cost to you.
Your Responsibilities
It is your responsibility while you are in the hospital@home program to do the following:
- Be home from work and take adequate rest as suggested by the hospital@home doctor and nurse.
- Provide your own meals or arrange for someone to provide meals for you.
- Keep any medications or equipment secure and away from children.
- Organise alternative arrangements for any household duties that you normally attend to.
- Follow the treatment prescribed by your doctor or recommended by your nurse.
Eligibility
A referral needs to be made by your general practitioner, a doctor from an emergency department or your medical specialist.
The doctor may suggest hospital@home to you as an alternative to being in hospital. This means instead of being admitted to a hospital ward, you have the opportunity to receive the same care in the comfort and privacy of your own home.
If you believe that you should not receive care in your home, please advise your nurse immediately.
Once you have agreed to be admitted to hospital@home, an assessment will be undertaken by a hospital@home nurse in your home. This assessment ensures that hospital@home can provide you with the care, security, comfort and safety that you need.
