Nursing
Services include general nursing care and depending on your location and availability, we can also offer monitoring of medication and rehabilitation to specialised procedures such as hospital in the home programs, intravenous therapy, wound care, stomal therapy and the managing of continence and diabetes.
Our nurses can access expert advice and support from clinical nurse consultants who are nurses that specialise in particular areas such as wound care, infection control, gerontology, acute care and continence.

Post Acute Care
Post Acute Care is nurse managed and available 24 hours a day, seven days a week providing care in the immediate post discharge period from hospital or surgical or medical intervention.
Community Nursing
Community Nursing provides you with an alternative if you need to be admitted into a public hospital for sub acute interventions.
hospital@home
hospital@home is a program that can provide you with home based acute care. The level of care is equivalent to being in hospital, however it is provided in the comfort of your own home.
Home Oxygen
Silver Chain is funded by the Department of Health to supply oxygen to you in your home, if you live within the Perth metropolitan area.
