Independence Programs
 
Personal Enablement Program (PEP)
 
Silver Chain's PEP assists you to regain and boost your well being after a period of time in hospital.
 
PEP gives you the encouragement and support you need to get up and go again, by providing you with the means to do things yourself.

PEP is a home based program which operates across the Perth metropolitan area and in selected country areas. 
 
During PEP your normal home care services may continue or Silver Chain will either provide the services or work closely with your current provider.
 
PEP runs for up to eight weeks and is delivered by a multidisciplinary team comprising of:
  • Registered nurses.
  • Occupational therapists.
  • Physiotherapists.
  • Therapy assistants.
  • Care aides (personal care).
  • Domestic assistants.

The PEP program aims to:

  • Assist you to achieve your highest level of independence.
  • Reduce or eliminate your need for ongoing services or readmission to hospital.
  • Prevent or delay any functional decline and promote healthy ageing.

Home Independence Program (HIP)

HIP is an innovative service that aims to improve your overall health and independence if you are over 65 years of age.

Activities include:

  • Learning new ways of approaching personal and daily activities that have become more difficult, so that you can manage your own activities without the need for assistance.
  • Using equipment that can assist with personal care and day to day activities.
  • Safely regaining confidence with managing day to day tasks, especially after a fall or an illness.
  • Learning how to reduce the risk of falling, both at home and outside.
  • Providing a home exercise program that helps to improve strength, balance and increases overall physical activity levels.
  • Education about healthy and successful ageing.
  • Looking into community based activities that suit your individual interests.

Social Enablement Program

Silver Chain has recently set up a new 12 week program for existing clients. The Social Enablement Program is about supporting you to rebuild your social networks.

With the help of a trained volunteer, we assist you to focus on social goals and then take steps to achieve them.  We will support you to pursue personal interests and to socialise with people you want to be with.

You might like to try something new, learn new skills, or take up a hobby or interest that you enjoy.  You might like to join a club, learn how to use a computer, get down to the shops or out in the garden.

Whatever your interests are we assist you in taking those initial steps and support you in building your confidence for living and socialising in the community as independently as possible.
