Disability Services

Silver Chain provides a combination of in home services to support people with disabilities funded under Disability Services, to live as independently as possible.
 
These services are available in the metropolitan area and some country locations.
 
We can assess your needs, provide and co-ordinate a range of services including nursing care and also arrange respite opportunities for family members.  In many cases, support can be tailored to suit your individual needs and preferences.
 
In order to assist you, Silver Chain will:
  • Meet with you to assess and prioritise your needs.
  • Negotiate and arrange the services you require.
  • Advocate on your behalf, if  required.
  • Be responsible for employing, training and supervising your support workers.
  • Co-ordinate support worker rosters, any changes to your support arrangements if or when required.
  • Oversee and manage your funding.

Our relationship with the Disability Services Commission (DSC) enhances the services provided to support people with disabilities and their carers.  With the ongoing commitment of the DSC, we are able to provide respite services to you, your family and carers to ensure your quality of life and care needs are met.

Our staff also benefit through specialist training funded by the DSC.

CATCH Program 
 
CATCH stands for Children who are dependent on Technology and Cared for by their families at Home.  CATCH is an innovative program which is the product of a collaboration between Silver Chain, the Womens and Childrens Health Service, the Red Cross, the Disability Services Commission and the Department of Community Development.
 
CATCH is designed to assist children who require the help of technology to meet their respiratory needs to live at home.  CATCH provides specially trained support workers to attend to their needs.
