Aged Care Packages

Community Aged Care Packages (CACP) and Extended Aged Care at Home (EACH) are flexible health care and support packages which enable people to remain in their own home when they would otherwise require admission to a residential care facility.
 
CACP and EACH offer holistic care delivered in your home and tailored through negotiation to meet your needs.  Silver Chain can also link you to other appropriate services and activities in the community.
 
To be eligible for CACP or EACH, you need to be eligible to enter an aged care facility. Therefore, your needs must be assessed by a member of the Aged Care Assessment Team (ACAT).  ACAT can meet with you in your own home or in hospital to determine whether CACP or EACH is suitable.
 
Your local ACAT might include a doctor, nurse, social worker, occupational therapist or physiotherapist.

Examples of services available through CACP and EACH include:
  • Personal care such as bathing or dressing.
  • Meals, laundry and short term respite.
  • Transport, shopping and social activities.
  • Housework, gardening and home maintenance.
  • 24 hour on call emergency assistance.

The advantages of CACP and EACH are the flexibility.  These packages can assist you to maintain your independence with the security and support of your family and friends.
