One of our friendly Customer Operations Representatives can advise what services you can access and are eligible for. This can include both new and additional services.
Our funding contracts require us to collect fees for some services. If you are required to pay a fee this will be discussed with you before you receive the service.
Payment may be made by any of the following methods:
- Direct debit.
- Cheque or money order.
- Credit card over the telephone by calling our Customer Operations Centre on (08) 9242 0242.
To ensure a safe working environment our staff members are not permitted to accept payment from you.
Silver Chain complies with the National Privacy Principles in the Privacy Amendment (Private Sector) Act 2000. Further information is available within our

Part of the assessment process is to determine eligibility for a variety of programs including Home and Community Care services.
Our Customer Operations Representatives will determine your eligibility by asking you a number of questions to identify your functional capability in every day living activities.
The assessment process also includes:
- Collecting information to determine your eligibility for programs and services.
- Identifying the level of care that you require to enable you to continue living in your community.
- Allocating the appropriate level of care that you require.

Do I need to be referred by a doctor or hospital to receive Silver Chain services?
Anyone can make a referral to access Silver Chain services. You can refer yourself for services, or you may be referred by a family member, carer, general practitioner, hospital or other health service.
To access our Hospice Care Service, a referral must be made by a medical practitioner.

How do I refer myself to receive Silver Chain services?
We prefer referrals by telephone Monday to Friday between 9:00am and 5:00pm. However referrals to our services can be made by facsimile 24 hours a day, seven days a week on (08) 9444 7265.

Do I have to be a certain age to receive services?
We offer a diverse range of services to you no matter how old you are.

Does Silver Chain service the whole of Western Australia?
Silver Chain provides services from Carnarvon in the north, to Walpole in the south and Eucla in the east.

Does Silver Chain offer an on call service?
Silver Chain provides a 24 hour, seven day on call service for our hospice, hospital@home, Post Acute and Community Nursing programs. This service can assist you if you need support and you are a client receiving one of these services.
We also provide a 24 hour, seven day on call accident and emergency health care service if you are in a remote location. This service can assist you if you require urgent medical treatment due to an emergency health problem or accident.

How can I support Silver Chain?
You can support Silver Chain by becoming a volunteer or by making a donation.
Volunteers
We have 18 branch committees that expand across Western Australia who play an integral role in supporting Silver Chain by raising funds, providing valuable feedback on the impact of services and identifying needs in their communities.
In addition, we have over 660 volunteers who enhance the quality of service we provide to our clients and their families through a range of activities including providing companionship and support, leisure assistance, shopping, errands and assistance at our events and displays.
Volunteers are critical to Silver Chain's success and we thank them all for their continued and invaluable support.

Making a Donation to Silver Chain
While Silver Chain receives funding from the Commonwealth and State Government which allows us to provide our services, donations and bequests help us to provide additional services, higher levels of care and appropriate infrastructure to support our services.
The work we do cannot be done alone. With your support and that of the community, we are able to continue the high standard of care that has become synonymous with Silver Chain.
